Office and Billing Policies
Insurance Eva Hosseinion accepts:
Eva Hosseinion is in network with most insurance plans, but you need to call or visit your insurance website to confirm, searching by her last name only: Hosseinion.
Eva Hosseinion is NOT taking any new Oregon Health Plan, Care Oregon, or Healthshare patients.
Eva is currently out of network for CHP (check this page for updates).
Missed Appointment & Late Arrival Fee:
There is a $60 charge for missed appointments not cancelled 24 hours in advance (unless there are extenuating circumstances)
Arrival for an appointment late by 20 minutes or more will be considered a missed appointment
Phone calls & Text Messages:
-For administrative questions, please leave a message on my office voicemail 503-893-9418, which is checked every couple days. Your call will be returned; this line CAN receive text messages as well.
-If you need to be seen in an appointment urgently, and there is no availability on my online schedule, you should call/text our office line at 503-893-9418.
Email & Text Messages:
-By making your appointment, you consent to receiving secure email and online communications from us -You can email our medical assistant at ma@elderflowermedicine.com
Appointments:
All appointments are scheduled online through the link on www.elderflowermedicine.com
If you need to be seen in an appointment urgently, and there is no availability on my online schedule, you should call/text our office line 503-893-9418.
Billing Policies:
If you don’t have health insurance, you may pay for your visit at the time of service with cash, check, or credit card.
Copay is paid at time of visit, the bill is sent to the insurance.
